What Role Does Protein Play in Your Bone Density? 🦴🤔

I have worked with multiple clients whose primary goal was to regain bone mineral density and support bone quality following the identification of osteopenia or a diagnosis of osteoporosis.


And while calcium and vitamin D understandably get a lot of attention when we talk about bone health, protein deserves consideration too.


Before getting into the research, a quick primer on some of the terminology.


Understanding Bone Mineral Density and Bone Quality

What Is Bone Mineral Density?

Bone Mineral Density (BMD) is the amount of mineral contained within a given volume of bone.


It is usually measured using a DEXA scan, which produces:


  • T-score: a comparison with the bone density of healthy young adults

  • Z-score: a comparison with people of a similar age


What Is Bone Quality?

Bone quality incorporates BMD, but also considers the architecture and structure of the bone itself.

This internal lattice structure influences bone strength and fragility.


Bone quality can be assessed using techniques such as REMS (Radiofrequency Echographic Multi-Spectrometry).


Osteopenia vs Osteoporosis

Osteopenia refers to bone density that is lower than the expected healthy range but does not meet the diagnostic threshold for osteoporosis.


Osteoporosis is a diagnosed bone disease characterised by reduced bone strength and increased fracture risk.

Protein and Bone Density: What Does the New Evidence Show?

A recent longitudinal observational study examined women undergoing treatment for low bone density and identified an interesting relationship between dietary protein, calcium intake and the effectiveness of antiresorptive medication.


The researchers found that:


1️⃣ Total calcium intake was not associated with an effect on bone outcomes.


2️⃣ Calcium supplement use was also not associated with an effect.


But here is the interesting bit.


3️⃣ In women consuming less than 800 mg of calcium per day AND less than 0.8 g of protein per kg of bodyweight per day, the effectiveness of their antiresorptive bone medication was reduced.


This is an important finding because it suggests that adequate nutrition may help support the effectiveness of medical treatment.


It does not mean that protein or calcium can replace medication.


Why Does Protein Matter for Bone Health?

The finding is also consistent with previous research.


As the study authors noted, systematic reviews and meta-analyses have demonstrated positive associations between protein intake and bone mineral density at the:

  • Hip

  • Lumbar spine

  • Total body


Protein is important not only because bone itself contains a substantial protein matrix, but also because maintaining muscle mass is critical for supporting movement, strength and physical function.


Muscle and bone health are closely interconnected.


Better muscle strength can support balance and physical function, while resistance and impact exercise provide mechanical stimuli that help maintain bone strength.


Translating the Research Into Practice?


Medication Still Matters

For someone with osteoporosis or significant bone loss, making use of appropriate medical treatment is critical.


Nutrition and lifestyle medicine can support efforts to maintain or improve bone density and quality, but the available medications can be highly effective.


For this reason, I always encourage clients with osteoporosis or clinically significant bone loss to work with their GP and other relevant healthcare professionals as part of their overall strategy.


The goal is integrated care, rather than choosing between medication and lifestyle.


Building a Comprehensive Bone Health Strategy

Once appropriate medical care is in place, a personalised strategy can address the nutritional and lifestyle factors that support bone and musculoskeletal health.


This may include:

🥩 Adequate dietary protein

🥦 A nutrient-dense dietary pattern, with an emphasis on a wide variety of whole foods

🍶 Adequate calcium, vitamin D3, vitamin K2 and magnesium, based on individual needs

🥛 Personalised calcium supplementation, where appropriate and in context of total dietary intake

🪢 Collagen peptides, such as FORTIBONE, where appropriate

🏋️‍♂️ Progressive resistance training

🤸‍♂️ Multi-directional impact exercise, where appropriate for the individual's bone health and fracture risk

☀️ Consistent daily daylight exposure, supporting healthy vitamin D status and circadian rhythm

😴 High-quality sleep and effective stress management


The important part is bringing these elements together into something that is personalised, practical and sustainable.


Bone Health Is About More Than Bone Density

Think Long Term

Bone density and bone quality are critically important enablers of long-term physical function and independence.


But they don't exist in isolation.

Maintaining muscle mass, strength, balance, mobility and metabolic health alongside bone health creates a much stronger foundation for healthy ageing.


The aim isn't simply to improve a number on a DEXA report.


It's to build and maintain a body that remains strong, resilient and functional throughout life.
